PREPARING FOR YOUR APPOINTMENT:
Entrance located on 137th Street, just off 96th Avenue. Drop-off is available in the back of the building, with a 15 minute time limit for no charge.

PREPARING FOR YOUR APPOINTMENT:
- If your referring physician has ordered any x-rays, please have these done at any Fraser Health facility. A list of Fraser Health Radiology Clinics can be found HERE
- Prior to your office visit, please take a few minutes to fill out the patient questionnaire that has been sent to you by our office. If you did not recieve a questionnaire, please contact our office and we will forward one to you.
- Please bring your BC Health Services Card and any other insurance cards/information you wish us to have.
- Know your family physician so copies of your visit notes can be forwarded to them.
- If you have a workplace injury and have a WCB claim please advise our office.
- Please bring a list of medications you are currently taking.
- Please bring any reports and/or CD's of any imaging you have had done (x-rays, MRI, CT Scan) pertaining to your current issue.
- For knee and hip exams, please wear or bring shorts (or skirt) or pants/trousers that can easily be rolled up above the knee. For wrist and elbow exams please wear a short sleeve shirt or top.
- INTERPRETERS Unfortunately, we are not able to provide an interpreter for your visit. If you have difficulty communicating effectively in english, we strongly suggest bringing a professional interpreter or family member who has a solid understanding of english. This will ensure we can take an accurate history and have a better understanding of your problem, so as to better able to treat you. If we are unable to effectively communicate with you, we may need to re-schedule your appointment for another day.
